1.) You're going to need a canvas that is 850x200.
2.) Create 4 layers like so.
To make yours look like mine, you'll need these renders;
3.) No you're going to want to select Layer 1, which should be at the bottom, then you're going to click File->Place and select this image; after you place it, rasterize it.
Now stretch it so that it covers the entire canvas, don't worry about it getting distorted, its already a big image.
4.) Repeat the same process with this image;
http://i.imgur.com/sttlA.jpg . This time though, stretch it to the same size as the canvas or close to it. Now what you want to do is right-click on Layer 2, and click on "Blending Options". Lower the opacity to about 13% so you gt the texture of the smoke on top of the background and it darkens it a bit. Repeat with next image.
What you have now should look something like this, mine is a little darker.
5.) Find a font that you like, if you want to use the font I did, it can be found
here. Now make a layer for each letter in the word your going to put in here. I.E. I made 5 layers. Now you're going to get the
and use it to type the first letter of the word you want. If you type and see nothing, try dropping the font size. I'm using 4pt font and normal size.
Now that you have your first letter typed, click the eye dropper and click on the color right above the letter, right click on the layer of the letter and go back up to Blending Options. Again turn the opacity down, this time to 18. You are also going to want to have these;
Repeat that process for the remaining letters, adjusting the opacity for each letter.
Congrats! You're done and should now have something like this.
