[SOLVED] Nudges not working correctly

I'm on the white theme, and nudges seem to not be working correctly.

The little square doesn't has the "0" in it, or any other number.
And when I click it, nothing comes up in window and it stays there.

it broke the site so it got disabled while it is getting fixed

Same thing happens on the black theme.

It will be disabled until we fix the problem, it's causing the server to crash.
